COMLEX Level 1 and USMLE Step 1 test overlapping biomedical science, but COMLEX layers osteopathic principles and practice on top of it in a way that catches some students off guard if they've been studying from Step 1-focused material exclusively.
OMM isn't a small add-on
Osteopathic manipulative medicine questions show up throughout the exam, not in an isolated section. Treating OMM review as an afterthought late in prep tends to leave a real gap on test day.
The clinical framing is a bit different
COMLEX vignettes often lean more heavily into primary care and osteopathic philosophy in how questions are framed, even when the underlying pathology is identical. Practicing with COMLEX-style questions specifically helps that framing feel familiar rather than distracting.
Decide early if you're taking both exams
Some students sit for both COMLEX and Step 1. If that's the plan, it changes how prep time should be split, since cramming both into the same narrow window rarely serves either exam well.
